This complaint form provides students with a means to submit a formal written complaint.
This form does not replace current and existing complaint resolution procedures as outlined in the Student Handbook, but is available to take submissions regarding any area on campus for which no other specific process exists, in addition to any other complaints. Any and all complaints submitted via this form are considered official and will be acted on.
After submitting the form, you will receive confirmation your complaint was received within 48 hours. You will be contacted by the appropriate office in a timely manner with the information requested, or with a description of the resolution process appropriate to your specific submission.
