Verification Documents

The U.S. Department of Education selects a number of FAFSA applications each year for a process called verification.  This means that financial aid officers must verify certain data elements on the FAFSA.  To do so, students and parents are required to submit certain documents to the Financial Aid Office.  Students whose FAFSA application has been selected for verification may need to complete one or more (but generally not all) of the following documents.  Please contact the Financial Aid Office for more information regarding which items are required.

Dependent verification worksheet.  To be completed by dependent students and their parent(s).

Independent verification worksheet.   To be completed by independent students (and their spouse, if applicable).

High school completion status verification. Used to determine a student’s high school completion status.

Identity and educational purpose verification.  Used to verify a student’s identity.

Parent W-2 form(s) – A copy of all W-2 forms for the parent(s)

Student W-2 form(s) – A copy of all W-2 forms for the student

Proof of Earnings Statement – Used to document earnings if a student/parent did not receive a W-2 form for income earned from work (must be signed and dated).

Parent Tax Return Transcript – An official tax return transcript that must be obtained from the IRS via the IRS website.

Student Tax Return Transcript – An official tax return transcript that must be obtained from the IRS via the IRS website.
