Download this form if you need to increase or decrease the number of credits you want to earn for your internship.

Required Forms to be TURNED IN for Credit Internships: (these must all be turned in at the completion of the internship)

Download form and email this evaluation to the Internship Coordinator at the completion of your internship. - Supervisor Internship Evaluation Form
Download this form if you need an additional copy for your On-Site Supervisor to fill out. Please have them fill out the front and back of the form. This is turned into your Faculty Sponsor as part of your grade - Log of Activities
Download this form and fill it out during your credit internship. This is turned into your to your Faculty Sponsor as part of your grade.
