Undergraduate Procedures
- Instructions for completing Application for Degree
- Major Program Requirements - Select the catalog year in which you declared your major and then select the specific form.
- Minor Program Requirements - Select the catalog year in which you declared your minor and then select the specific form. If the specific form is not listed, print this minor form.
Masters Procedures
- Degree Requirements - Select the catalog year in which you began your program and then select the form specific for your degree -- look for either Master of Arts or Master of Education in the listings.
