Undergraduate Procedures

  • Instructions for completing Application for Degree
  • Major Program Requirements - Select the catalog year in which you declared your major and then select the form specific for your major.
  • Minor Program Requirements - If you have declared a departmental minor, complete this form.  For interdisciplinary minors, refer to Major Program Requirements.


Masters Procedures

  • Degree Requirements - Select the catalog year in which you began your program and then select the form specific for your degree -- look for either Master of Arts or Master of Education in the listings.
