Pre-Participation Forms (PDF) and Online Forms:
Prior to participating on a Randolph College Athletic team, all athletes must provide the Athletic Training Department with personal information to assist and insure treatments that meet each student athlete’s needs.
FIRST YEAR STUDENT ATHLETES AND TRANSFER STUDENT ATHLETES:
1.) Medical paperwork to submit via mail:
If you are a new student athlete use the First Year and Transfer Student-Athlete Medical Paperwork pdf. at the link below. Please download, print, complete, sign and mail to us by August 1, 2023:
New Athlete Forms
You are encouraged to complete the medical history form and take it with you to your physician for your physical exam for review. Be sure to get the form back from the physician!
2.) Forms to be completed online:
Along with the paper forms above, you are required to complete online forms as well. To expedite this process, RC utilizes SportsWare On Line (SWOL), an online data entry system. To log on to the system, you will need your current RC email address (ssmith@randolphcollege.edu). To enter your information, visit www.swol123.net. You can also follow directions in the “SWOL Introduction Letter to Randolph Athletes” link:

The first time you visit the SWOL website you will need to enter your Randolph College email address and click Get Password. Please have this completed by August 1, 2023.
You will be using this system for your entire stay at Randolph, so keep track of your password that you create.
RETURNER STUDENT ATHLETES
1.) Medical paperwork to submit via mail:

2.) Forms to be completed online:
Log onto SportsWare OnLine www.swol123.net. When you visit the website you will need to update your general information, insurance information, upload your insurance card, etc. You do not need to update your medical history on SWOL.
Online Forms are to be completed online by August 1, 2023.
Reminder: be aware that all athletes must have everything completed and turned in as requested before they will be cleared to participate in any team training, practices, lifts, games, etc.
ADHD Documentation (FOR ALL STUDENT-ATHLETES)
Any student-athletes taking prescription ADHD medications MUST read the attached letter and fill out the attached ADHD form below.
